What is natural desiccated thyroid?
Natural desiccated thyroid — also called NDT, desiccated thyroid extract, or DTE — is a prescription thyroid hormone medication derived from porcine (pig) thyroid glands. It contains both T4 (thyroxine) and T3 (triiodothyronine), along with smaller amounts of T1, T2, calcitonin, and other thyroid-derived compounds. Common branded versions include Armour Thyroid, Np Thyroid, and Wp Thyroid; compounded versions are also available.
NDT has been used to treat Hypothyroidism since the late 1800s, long before synthetic levothyroxine was developed. Today it is considered a legitimate option — particularly for patients with Hashimotos Thyroiditis or post-surgical hypothyroidism who do not feel well on T4 alone.
How does NDT work?
A standard 60 mg (1 grain) tablet of NDT contains roughly 38 mcg of T4 and 9 mcg of T3, in a fixed ratio of about 4:1. Once swallowed, the T4 acts as a steady reservoir while the T3 delivers a more immediate signal at the cell. T3 binds directly to thyroid hormone receptors and activates the genes responsible for energy production, body temperature, metabolism, mood, and cognition.
Because NDT contains active T3, it suppresses TSH more strongly than T4-only therapy. Monitoring requires a full panel — TSH, Free T4, and Free T3 — and is interpreted alongside symptoms rather than TSH in isolation.
When is it prescribed?
NDT is most often prescribed for:
- Women with hypothyroidism who feel unwell on Levoxyl, Synthroid, or generic levothyroxine despite a normal TSH.
- Patients with documented poor T4-to-T3 conversion.
- Post-thyroidectomy and post-Radioactive Iodine Ablation patients.
- Patients who prefer a single tablet that contains both hormones rather than separate T4 and Liothyronine.
Patient considerations
NDT is taken once daily — sometimes split — on an empty stomach, away from coffee, calcium, iron, and biotin. Side effects almost always reflect too much T3: palpitations, jitteriness, insomnia, anxiety, tremor, sweating, and unintended weight loss. Long-term over-replacement can accelerate bone loss and increase the risk of atrial fibrillation, especially in older women. NDT is not appropriate for untreated adrenal insufficiency, uncorrected hyperthyroidism, or unstable cardiac conditions, and it should be used carefully in pregnancy.
NDT contains pork-derived ingredients, which is a consideration for vegetarians, vegans, and patients with religious restrictions. NDT is also not gluten-free in every formulation — patients with celiac disease or Hashimoto's should confirm with the manufacturer.

Is natural desiccated thyroid better than levothyroxine?
Neither is universally better. Levothyroxine works well for many patients, while others feel significantly better on NDT because it provides T3 alongside T4 in a fixed ratio. The right choice depends on labs, symptoms, conversion patterns, and individual response. Some women try T4 first and switch; others do best on a custom combination of T4 and T3. The decision should be based on a full thyroid panel and how a patient actually feels — not on philosophy alone.
Does NDT have a fixed T4-to-T3 ratio?
Yes. NDT delivers roughly four parts T4 to one part T3, mirroring the natural ratio in pig thyroid tissue. That ratio is higher in T3 than the human thyroid produces (closer to 14:1), which is why some patients feel great on NDT and others feel over-stimulated. Patients who need a different balance — for example, more T4 with a smaller, more targeted T3 dose — often do better on a combination of separate T4 and liothyronine, or a compounded preparation.
Is NDT safe long term?
When properly dosed and monitored, NDT is considered safe long term. The main risks come from over-replacement: bone loss and atrial fibrillation, particularly in postmenopausal women and patients with cardiac history. Regular labs — TSH, Free T4, Free T3 — and bone-health monitoring matter. NDT is not a shortcut around clinical oversight, and self-dosing or buying it without a prescription is risky. With a thoughtful clinician and consistent follow-up, many women stay on NDT comfortably for decades.
